Prescriptions:

By Ohio law, all pets must receive an exam by one of our licensed veterinarians in the last 12 months in order for the veterinarian to prescribe medication to the pet. Medications cannot be prescribed to pets who do not have a valid veterinary client-patient relationship with one of Middletown Animal Hospital’s veterinarians. A canine patient must have a current negative heartworm test on file in order to dispense heartworm preventative. Some preventatives may require additional heartworm testing to be covered by a manufacturer guarantee.

Payment Options:

Middletown Animal Hospital tries to provide high-quality care at fair prices. We are happy to provide you with a detailed estimate prior to treatments for your pet. We accept cash, Mastercard, Visa, American Express, and Discover cards. We require payment in full at the time of service.

A deposit will be required if a pet needs hospitalization and the remainder of the balance must be paid in full at the time of discharge. Unfortunately, we are unable to offer in-house payment plans, but we do accept Care Credit and VetBilling which allow convenient monthly payment options.

Appointment Deposits:

We require a deposit for all new clients scheduling appointments. The cost of the deposit will go toward the cost of the appointment. If the appointment is considered a no-show, canceled, or rescheduled, the deposit is forfeited.

We require a deposit for clients scheduling multiple pets for the same appointment time. The cost of the deposit will go toward the cost of the appointment. If the appointment is canceled or rescheduled with less than 48 hours’ notice, the deposit is forfeited.

Deposits will be required for all appointments if more than two cancellations with less than 48 hours’ notice or two no-show appointments occur. If the appointment is canceled or rescheduled with less than 48 hours’ notice, the deposit is forfeited.

Surgical Appointments:

A non-refundable payment is required for all surgical procedures at the time of scheduling in order to reserve the appointment. The deposit is applied to the procedure’s price. Surgery must be postponed or canceled 3 working days before the procedure to avoid forfeiting the deposit.
